Affidavit
of Publication
Whenever
an Affidavit of Publication for a "Trade Name" is
filed, the newspaper will send three (3) copies of the Affidavit
to the individual who filed. The individual is responsible to
send one copy to the Secretary of State, one copy to the Douglas
County Clerk and one copy is for their records.
An original
copy of the Affidavit of Publication must be filed with the
Douglas County Clerk's office with a filing fee of $5.00. After
filing, a photocopy of the Affidavit, showing the date filed,
will be returned to the sender to verify the filing.
Corporations
Corporation
papers are no longer filed with the County Clerk's office. Information
on how to file a corporation name can be obtained from the Nebraska
Secretary of State's Office at:

Military
Discharge Papers
Veterans
are encouraged to record their discharge papers with the County
Clerk for safekeeping and proof of service for Veterans Administration
Benefits. These records are filed as a courtesy to our veterans.
There is no charge to file a DD Form 214 or to issue certified
copies of military records. A person can file their military
discharge paper with our office at 1819 Farnam Street, Room
H08, Omaha, NE 68183.
If a copy
of a military discharge record is needed, the veteran will need
to provide his/her full name at the time of enlistment, his/her
social security number, and date of birth. Military discharge
records can only be provided to the veteran and cannot be
provided to relatives of the veteran. If the veteran is deceased
or unable to come in person, please contact the Douglas County
Veterans Service Office at 444-7180.
Partnership
& Sole Proprietorship
As of January 2003,
Business & Sole Proprietorships are no longer filed with the
Douglas County Clerk’s Office. They are filed with the Nebraska
Secretary of State’s Office. Please contact them at 402-471-4079

Power
of Attorney
A power
of attorney form may be filed at the Douglas County Clerk's
office for a fee of $5.00 per page, but it is not required to
be filed. This will be filed as a miscellaneous filing in the
County Clerk's office.
